To join the practice, simply ask at reception for a Registration Form. If you want to be able to use EMIS Access online to book appointments, check results, and order repeat prescriptions, then ask for an EMIS Access Registration Form from reception.
Complete and return the form, with your NHS Medical Card if you have it (if you don't, the reception staff can get a new one for you). Reception will let you know what other paperwork might be required (eg: your passport or driving license to confirm your identity).
We will then arrange for your notes to be sent from your old GP, and you will be asked to come and see our practice nurse for a registration medical.
Please note that registration takes 2-4 days to complete. Until we receive your NHS number from Practitioner Services we will not be able to order specialist referrals.
